At Dental Arts of Bedford it is our goal to make your experience as comfortable and streamlined as possible.  For this reason we have made our New Patient forms available to help us serve you more efficiently.  You will find our two New Patient forms available below.  Please print them, fill them both out and bring them with you to your first appointment.  (You may also mail them in ahead of your appointment if this is more convenient, however, please be aware that we must receive them by the day of your appointment). 

If you do not wish to print the forms out, you may also fill them out at the time of your appointment.  To do this, we ask that you arrive 15 minutes prior to your initial appointment to enable us to serve you promptly at your reserved time.
